Short-Term Stay After Hospital Admission
Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

In-Patient Residential Stay
Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

[review] => One year ago, the independent, living on her own, lively matriarch of our family suddenly ave unexpectedly began to experience a sharp decline into dementia. Her health overnight declined so badly that we were sure we were seeing our last days with her. Reluctantly we as a family decided to place her in the care of Dr. Patel and the staff of Palm City Nursing and Rehab, and nearly right away she began eating, walking (with assistance), and conducting daily activities again, as opposed to barely being able to keep her eyes open just a week or two before. Today, she is still in need of care, but it is due to the communication, care, and services provided at this facility that she is with us and doing so well. We cannot thank the dedicated staff enough for caring for her, and becoming apart of our family. Thank you all! - Crystal

Which makes me feel good to know that my loved one would be staying in a well kept place. The Executive Director April was there to answer any questions my family and I had and was very educated on her facility, staff, and position. [review] => Phenomenal PT Department!! Have been to South Florida Orthopaedics as well as Cleveland Clinic PT and neither one can compare to Palm City Nursing & Rehab Center. Cleveland Clinic could only get me in sporadically and said they would call me if they had any cancellations after I started going there, even though my Dr. prescribed 3x per week. I was seen at Palm City Nursing and Rehab within 2 days of contacting them and am being seen 3x per week with excellent results. My physical therapist, Christie, has exceptional interpersonal skills along with outstanding PT knowledge . I highly recommend this facility!!!

[review] => Don't go here or send someone you care about. I was a patient there for 2 weeks after a total hip replacement. Most of it was just awful. Getting the aids and nurses to answer the call light in less than 30 minutes was impossible. I developed a UTI because I was left in wet briefs so often. My brothers had to call several times and tell them to attend to my needs. Then it came with a negative attitude. I didn't have a shower once! Nursing made mistakes with my meds. many times.
My meal trays came with dirty glasses and utensils. The third shift charted they had come in and turned me, when they didn't. After reporting this to a supervisor, I thought things would improve. They didn't. There was only one cna I looked forward to seeing because she cared and treated me with dignity. Physical therapy was ok. Occupational therapy was awesome. Those are the only two positive things to say. There are several other things I could post, but I just hope you don't go near this facility.

My loved one was given the incorrect medication twice, didn't have a shower or a change of clothes for three days, and pressing the call button is a joke. [review] => My father is probably the hardest resident that the staff has to deal with. He is a very difficult person. The center and there leadership have gone out of there way to help and accommodate him in every way. They are in constant contact at least weekly on how he is doing. I work out of town during the week and it is a great relief that they go the extra mile to make sure my loved one is getting great care. The facility is close to my home and makes it easier for family to visit. I would recommend Palm City Nursing and Rehabilitation to any family facing the heartbreaking decision to have a love one placed into a nursing home.

[review] => This is the absolute worst nursing home i’ve ever been to. My poor grandfather was taken here for rehabilitation and came out barely alive. He was completely malnourished and had severe dehydration to the point where I needed to go in and call 911 because they refused to release him. When we asked them to call an ambulance they said they would in a little bit. The Nurse Practitioner was telling us he needed to go to Hospice because he was 98 years old and it was a part of life. Yet he’s 89 and walked into the facility on his own? My grandfather will now he bed ridden for the rest of his life and will not be able to enjoy his final years because of this disgusting nursing home. The people that work here should be arrested for the way they neglect these patients and this facility needs to be shut down before another family loses a loved one or comes close like we did. I also have a friend who lost her aunt because of the mistreatment of this place. Please DO NOT send any family members here if you care for them. I would not even give them a star if I could.

[review] => Palm City Rehab is awesome! My mom stayed here for about a month and I only have positive things to say and remember about her stay. The staff was impeccable! From Renee, Marie, Liphaite, Nurse Nicky, And (I’m so sorry/embarrassed that I took so long to write this post this review!!) my favorite Social worker/manager who was an absolute GEM from intake all the way through the time my mom was released, as well as the nice nurse with the red scrubs, and even the ladies at the front desk/intake- all the staff were super friendly, attentive, professional and VERY personable. They made her feel at home and her guests feel welcomed each and every time. They helped strengthen her through therapy and support until she was ready to go home. She was in escruciating pain and the staff was compassionate and integral in seeing her through her stay. The rooms were nice and the grounds clean and updated, but I can’t stress how much I would recommend Palm City rehab because of the staff. The team worked together and did an excellent, excellent job.

[review] => I received excellent service post operative for total knee replacement. The PT staff ensured that I reached maximum knee flexion. My orthopedic surgeon was amazed at the flexion and mobility that I achieved in Rehab.
Nursing staff members provided excellent care ensuring that I received all my medications and treatments in a timely manor.
I highly recommend this Rehabilitation Facility for all orthopedic postoperative patients. My total recovery time will be considerably shorter because of the PT program at this facility.

[review] => My loved one is a resident at this facility. While it does have a lot good points. The physical therapy and occupational teams are amazing. The bad seems to outweigh the good most of the time. The care isnt very consistent. ( sometimes good, sometimes bad) There are 2 or three aides that are very good. They care and seem to like their jobs. The nurses do what they have to. I have sat in the room for over an hour waiting for help to clean after a BM, or to get them ready for bed. I have had to clean them, feed her and bathe them because no one was around. I even had an aid come in while I was cleaning after a BM. She looked in, then turned around and walk out. It seems that when you suggest something to the nurse, it can take days for something to happen. I have even heard “oh I forgot”. During the weekdays from what I hear isn’t quite as bad. Maybe cause there is admin there, I don’t know. Nights and especially weekends are when I personally have experienced issues. The rooms aren’t CLEANED often enough. They do straighten them up daily. The bathroom smells of urine all the time. The food, in my opinion and my loved ones, is not good at all. It’s just sad that the elderly can’t get good care. They deserve better. I just pray for the ones that don’t have family to be there for them. Waiting to have my loved one moved. Praying for a better experience.

[review] => My elderly mother with chronic dementia has been at PC N&R for 6 months now. For the past 4 years she has been virtually comatose - not even acknowledging family or any other people. Since she has been here she is a different person - alert, responsive, answering simple questions and most surprisingly, looks at, and responds appropriately to people, family and her caretakers. She has been in a number of local facilities, depending on the level of care required, and our whole family has been delighted with the quality of care, social interaction, and care of the caregivers. The therapists are outstanding - as has been our experience with everyone there. Very pleased and have already recommended them to families needing care for elderly relatives. Thank you PC N&R!

[review] => My mother was at the Palm City Nursing and rehab facility. They made multiple mistakes at administering her meds. We met with the staff. Problems and incompetence continued. We had to monitor all the meds she was given and many times the giver didn't even know what they were. One evening an LPN after making a mistake handed me her meds and said you find her medicine I don't want to make another mistake. Another instance she was given two doses too close together which caused an overmedication missing a meal and rehab. The list goes on. Rehab department is excellent but you can't do rehab if you can't get meds right. We left our stay early to get rehab in the home and manage meds ourself. I can't imagine those who have no advocate with them at this facility. We will return for rehab but never for any other service. If it's the only place you can go please have an advocate with your loved one 24/7 during their stay.

[review] => Awesome rehab team! The facility is beautiful, clean, well maintained and the rehab department is fantastic! The nurses are friendly, helpful and caring and the food was great. I would highly recommend this facility and I will be returning when I have my other surgery.

[review] => No nursing home is perfect,however Palm City tries to present a homey atmosphere for its residents.
Food service is good and the rehab Dept is top notch. For the most part nursing staff provides caring and compassionate care. If I have concerns now and then, I find management very approachable .

[review] => TERRIBLE!!!! Dirty, dirty, dirty! My mother in law was here for about 1 week before she was taken to hospital by ambulance because these people were giving her so many medicines that she was constantly drugged up and all of her electrolytes were out of whack. I honestly think they would have killed her if she had stayed there any longer. Most, not all, but most of the caregivers (wouldn't call them nurses, most days there was only one RN on staff) were very uncaring and didn't want to even take the time to speak with you. DO NOT go here!!!!

REVIEWS
One year ago, the independent, living on her own, lively matriarch of our family suddenly ave unexpectedly began to experience a sharp decline into dementia. Her health overnight declined so badly that we were sure we were seeing our last days with her. Reluctantly we as a family decided to place her in the care of Dr. Patel and the staff of Palm City Nursing and Rehab, and nearly right away she began eating, walking (with assistance), and conducting daily activities again, as opposed to barely being able to keep her eyes open just a week or two before. Today, she is still in need of care, but it is due to the communication, care, and services provided at this facility that she is with us and doing so well. We cannot thank the dedicated staff enough for caring for her, and becoming apart of our family. Thank you all! - Crystal
